The following is required for each admission... 

1. Application:
   Submit an application with a $50 non-refundable application fee.  Applications are reviewed and processed as that are received. (application form)
2. Essential Requirements:
   Read the Essential Requirements for Hartford Hospital.  Return the completed signature page with application. (requirements form)
3. Transcripts:
   Submit all official high school and college transcripts.  Foreign transcripts need to be evaluated for U.S. equivalency by a foreign transcript evaluation agency.
4. References:
   Submit four reference forms to individuals such as teacher, counselor, employer, nurse, or doctor.  Individuals should not be relatives or personal friends. (reference forms)
5. Placement Exam:
   Qualified applicants will be scheduled to take a placement exam, to be given once a month from December to April.   A score of 75% or better must be achieved to continue in the application process.  After taking the placement exam, applicants will be contacted with results within 7 days.
6. Clinical Visit:
   Applicant will observe in the clinic for 4 hours to be scheduled by the admissions committee.
7. Interview:
   For all successfully completed applications, the admission committee and applicant will meet for an interview.  These will take place in April and August (if necessary).  Applicants will be notified with 14 days of the interview.
8. Physical Assessment:
   Occupational Health & Safety determines final acceptance. The student must pass pre-enrollment screening including substance abuse screening.
