Inter-Facility Transfers/Inpatient Admissions

All inter-facility transfers or patient referrals for inpatient admission are arranged through the Admissions Coordinator (during the day, Monday through Friday) or Nursing Coordinator Bed Manager (off-shift and weekends) at 860-545-1233 or 1-877-506-1233. The Admissions Coordinator/Bed Managers are all Registered Nurses. Placements for the Institute of Living are managed through their own intake coordinators at the IOL Assessment Center at 860-545-7200.
 
The Admissions Coordinator/Bed Managers will request if arrangements have been made to obtain an accepting MD at Hartford Hospital. In the event that no MD has been obtained to accept the patient, the Admissions Coordinator/Bed Manager will gather demographic information about the patient, a phone number for return call, and will contact attending MDs at Hartford Hospital to arrange acceptance of the patient to his/her service. Clinical information will also be gathered for the purpose of determining potential level of care and service needs.
 
The Admissions Coordinator/Bed Manager will call back the sending facility/caller with the name and contact number of an accepting MD at Hartford Hospital and gather the following information: 
  • Patient Name
  • Patient demographics (request for FAX of demographics sheet if available)
  • Patient’s Date of Birth
  • Patient’s diagnosis, symptoms, procedure required
  • Patient’s Insurance and guarantor information
NOTE: If authorization is necessary, the Admissions Coordinator/Bed Manager will request that the sending facility/referring MD obtain the authorization for transfer/admission to Hartford Hospital prior to the patient’s arrival at the hospital.
 
The Admissions Coordinator/Bed Managers determine unit assignment based upon above criteria.
 
Minors (defined here as patients less than 18 years of age) will be admitted to Connecticut Children’s Medical Center unless unique circumstances exist and special arrangements are made.
